NEW PATIENT

New Patient v. Established Patient

Everyone is a NEW Patient at Texas Family Wellness Clinic. Even if "Dr. G" has been your doctor for decades ~ you are still a new patient and we are thrilled to hear that you are going to follow him during this big, new chapter of his career, but there will be a process we must go through to establish you as a "regular" around here.  Please be patient with us, as our team is going to try to make this as smooth as possible for you & your family to transition over to Dr. Guevara's new place.  See the steps below you'll need to take to get established as a patient.


If you are a brand new patient, meaning you've never seen Dr. Guevara ever in your lifetime, we are happy to hear from you & you'll need to also complete the steps below ~ no special treatment for the newbies.


The only exception to scheduling a New Patient Establish Visit is if you are due for your Annual Wellness Visit (AWV).  We can do both in the AWV for you & get you established at the same time in one visit. If you cannot remember when you had your last AWV, you may call your insurance to ask, call the clinic location where you had it done or we can assist when you call to schedule an appointment. Your AWV should be fully covered by your insurance, if it's been a year since your last AWV. 

Problem Visit v. Establish Visit

Life happens.  We completely understand that sometimes the stars just don't align for you to get in to see us to Establish as a patient before you have a problem ~ need a refill on a medication, get sick, or need a referral.  We will still need to get you Established as a patient first and therefore you'll need to be prepared to have two separate appointments, maybe the same day or two days in a row. If you have a minor/acute issue we may be able to see you in-person to get "Established" & then set you up for a TeleMedicine Visit for that afternoon or the following day to take care of that acute issue.  We will try our very best to accommodate you during this process of transitioning over as a new patient.  

Schedule An Appointment

Since everyone is a new patient at Texas Family Wellness Clinic, you'll need to schedule a "new patient establish visit" to become "established" with the clinic.  This means you will need to have your medical records requested & available to us prior to your first visit.  Getting in for an "Establish" visit will allow us to essentially get "all of our ducks in a row" & prepare for when you do have a problem visit, such as a medication refill, medical need, or are sick, etc. The main focus from the physician's point-of-view for this visit is to have a list of all of your medical conditions, surgeries, along with the dates & who performed them, as well as a current medication list and allergies.  We will also need a list of all the specialists that you see & we will want to be sure that your family history is recorded accurately in your chart.  You'll need to be prepared to discuss your social behaviors, such as drinking & tobacco use, etc.


We will not accept walk-ins, so please schedule an appointment.


Before we can schedule your appointment, we want to be sure that we are in-network with your insurance.  


Please text a picture of your Driver's License and the front & back of your Insurance Card(s) to the Office Administrator at (361) 600-7018.  


Once your insurance is verified we will give you a call to schedule your very first appointment & answer any questions you may have. 

Before Your First Visit...

STEP 1

After you schedule your first visit you'll receive an email or text message to set-up your Patient Portal.  This is a great tool to see lab results & imaging results, schedule another appointment with a few clicks, update your info & insurance, etc.  We will need you to "Accept" all the New Patient Consent Forms & this can be done on your Patient Portal the very first time you log on. 


STEP 2

You will need to Request your Medical Records to be sent to our office.  We DO NOT have access to anybody's medical records.  You are a new patient at TFWC.  Everyone will have to request their medical records.  This is one of the most important things you can do prior to your visit because if we don't have any medical information you will have to arrive at least 20 minutes prior to your appointment time to answer a very thorough questionnaire to record all of your past medical history.   


You will need to download the Medical Record Request form, complete it & either email it back to us at  tfwcadmin@texasfamilywellnessclinic.com & we will submit it for you OR you can submit it directly via fax to the other clinic or drop it off in-person at their office. If you are currently seeing any specialists, such as a Cardiologist for a chronic long-term issue, please also send them this medical record release form.  (The other doctor's office you are submitting your Medical Record Request form to has two weeks by law to get us your records.  If you are under a time crunch, we strongly suggest that you drop off the form in-person & work out something with them to pick-up your records ASAP. If their office uses eClinicalWorks as their EMR Software, you can ask them to electronically submit your records for free through "P2P".)


Confirm Your Appointment

STEP 3

If you have provided us with a cell phone number, we will send you a text message about three days before the visit to confirm your appointment.  This will allow you to confirm that you are still going to make it to your appointment or you may cancel at that time.  After confirming your visit, you will then receive another text to "Check-in" for your appointment.  

Contactless Check-in

STEP 4

Due to the current COVID circumstances, our lobby is CLOSED. We ask that you complete your check-in prior to arriving for your appointment and/or after you've arrived in the parking lot.


Our Contactless Check-in feature allows you to do many things that you would normally do at the front desk, in-person with a staff member, such as verify your address, phone numbers, email, insurance information, sign our consent forms, and pay your co-pay and/or account balance.  All of these tasks can be completed via your smart device before you even step foot inside of our office ~ a few days before your visit.  Once you've completed your Contactless Check-in, all you will need to do is click on that same text message when you get to our office & tap the "Arrived" button.  This will notify our staff that you are literally here & ready to be seen by the doctor.  One of our staff members will call you when we have a room inside the clinic available for you.  


Depending on the reason for your visit, during the check-in process, you may be asked a series of questions regarding your current health, family history, signs & symptoms, etc.  These questions were specifically designed by Dr. Guevara to expedite your appointment for him to review prior to your visit.  The Contactless Check-in feature, if utilized to it's fullest by the patient, can provide you more time with the doctor & keep appointments on-time, without long wait times. It's also a great feature for parents & caregivers to complete prior to the  visit from the comfort of their own home.  


If you don't have time to finish your Check-in, it will remember where you left off & you can return to it later when you have the time.  We will have iPads available in the lobby to also use to check-in, but you'll need to arrive at least 10-15 minutes earlier than your visit time. 


If you don't have access to a smart device or just want to wait until your visit to check-in, you will need to arrive at least 15 minutes before your appointment time to complete the check-in process at our office.  This will require you to complete the New Patient Packet, which you can download now & bring it to your visit.  You can also mail it to us at 15406 Northwest Blvd., Ste. B, Robstown, TX 78380, or scan it & email it to tfwcadmin@texasfamilywellnessclinic.com.  If we can have your completed new patient packet & your medical records prior to your visit, it will make your visit run much smoother & quicker. Even if you complete your new patient packet prior to your visit, you will still be required to arrive at least 10 minutes earlier to complete any questionnaires designed specifically for your visit to expedite your appointment.  

Arrive Earlier than Your Appointment Time

If you are late to your appointment time, you will not be seen & will need to reschedule ~ even one minute late.  Everyone's time is valuable, not just the doctor's.  Unfortunately, if you are 6 mins late, and Sally is 4 minutes late, then Johnny is 5 minutes late... little Susie has to wait 15 minutes to see the Doctor.  If you need to cancel, please call us & try to cancel at least 24-hours before your appointment time to be sure you are not charged a $50.00 fee.  As a precautionary, just arrive 10-15 minutes early.  Sometimes we may need to grab extra information from you while you are there or we might have a question/issue to discuss with you, so it's always best to be early ~ especially when you are a new patient.  If we can take you back early to get your labs going or vitals documented, we certainly will.
