Thank you for your interest in wanting to become a new patient at our wellness clinic. If you have seen one of our Health Care Providers before at another clinic in the past, you will still be considered a New Patient at Texas Family Wellness Clinic. We have an Application Process to be considered as a new patient. The Application includes the submission of a copy of your Photo ID and the Front & Back of your insurance card. If we do not receive a copy of these items along with the application, your application will NOT be processed.
Over the past several years we have found that our approach and services are not everyone's "cup of tea". We require the application process to become a patient to be sure that we are going to be a good fit for you before establishing a patient / physician relationship. Our philosophy is mostly from a preventative and more natural approach and tend to be a bit different than most medical clinics. We aspire to assist patients with their health goals and be their partner in their health.
STEP 1
Click Here to complete the new patient application. You can attach a photo of your Photo ID and the front & back of your insurance card to the application OR text the photos to our New Patient Line at 361-360-3462 ~ whichever is most convenient for you. If we do not receive a copy of your photo ID and Insurance Card (front & back) along with the application, your application will NOT be processed.
***Do not call the New Patient Line, it is not a number that we answer ~ it is a back office line for patients to send in their insurance cards & photo IDs.
STEP 2
You'll receive a call and/or text message from us to let you know if your application has been denied or approved in about 7 to 10 business days. If it is denied, it is mostly because we are not accepting patients with specific insurance plans or insurance companies, eventhough we are in network with that insurance carrier. This can change at anytime by the business owner of the clinic. You are welcome to check back periodically by calling us at 361-232-4187. Another reason your application may be denied is because we do not prescribe Narcotic Medications and we conduct a national search on the prescription database for each applicant.
STEP 3
If approved, you'll receive a text message to set-up your Patient Portal. This is a great tool to see lab results & imaging results, visit summaries and much more with a few clicks, update your info & insurance, etc. We will need you to "Accept" all the New Patient Consent Forms & this can be done on your Patient Portal the very first time you log on.
STEP 4
After your application is approved, schedule your appointment by calling our office at 361-232-4187 during clinic hours, Monday - Saturday from 8am to 5pm. You MUST have a reason to be seen by the Provider. "Just to Establish" is not a reason for a doctor's visit. It will not be covered by Insurance. We do not administer "Wellness Check-ups" or "Annual Check-ups" at the very first New Patient Appointment.
STEP 5
Our staff will contact you via telephone to Confirm Your Appointment a few days before. Please be sure that you respond by calling or replying back to the text message. If you cancel your New Patient Appointment with less than a 24 hours notice of your appointment time or do not show up for your first appointment, we will not reschedule your new patient appointment and you will not become an established patient at our clinic.
Here are some tips to make sure you get the most out of your visit...
* Our Providers require that you bring all of your Medication Bottles, Vitamins, Supplements & Over-the-counter medications with you to your appointment ~ do not bring a list or photos.
* Bring a list of what your questions are & what issues you would like to discuss with the Provider to your visit. Write them down in order of priority. Due to time restraints, the Doctor may not get to all of them, but it is important for the Doctor to see your list & discuss them with you to come up with a plan of treatment.
* We do not accept cash & we do not accept American Express. All other forms of payment are accepted.
*Arrive 15 minutes before your scheduled appointment time. You will have paperwork to complete in-person that requires your signature. If you are late to your appointment, you will not be seen by a Provider.
*If you want to discuss recent labs or test results with the Provider, please bring those records with you to the appointment. We will NOT have access to any of your medical records at your first appointment.
*If you would like to discuss a Diagnostic Imaging result (MRI, X-Ray, CT Scan, etc), do not bring a CD-ROM, bring only the written report from the Radiologist. We do not look at CD-ROM/Films.
*If you see any specialists (Cardiologist), please bring a list of the names of the Specialty Doctors, the Clinic Name, & their clinic phone number. This will ensure that our staff is requesting your medical records from the correct Specialist.
*If your first visit is a follow-up from a Hospital or ER Visit, please be sure to request your medical records to be sent to our Clinic via fax at 800-990-5305. Our Providers cannot do much for you without seeing these records from the Hospital. Due to HIPPA Regulations, your signature is required to request your medical records. You may submit our Medical Record Request Form with your signature on your own or stop by our clinic at least 3 business days prior to your Appointment to sign this form, so that we can request the medical records on your behalf.
